Medical Record Department
Noble Heart Hospital’s Medical Records Department is dedicated to maintaining your medical records and keeping your health information private and secure in accordance with your rights as a patient, and with federal and state regulations.
We maintain hospital records for years following your last visit. These records include laboratory and radiology reports, emergency department visits, outpatient surgeries, and inpatient stays at Noble Heart Hospital. The Medical Records Department does not have access to individual physician office records.
How to obtain a copy of your medical record:
If you’ve been a patient at Noble Heart Hospital, you may obtain a copy of your record from the Medical Records Department, Release of Information Section (ROI). You’ll need to sign a Nobel Heart Hospital Authorization to Release Personal Health Information (PHI) Form and provide us with a photo ID for verification
